1000 people Project - National Cancer Survivorship Initiative

There is growing evidence that people living with cancer gain considerable benefit from becoming involved in managing their own condition. However, if they are to do this successfully they need the right information about their condition and the potential consequences of their treatment to better manage their ongoing needs post treatment. Livestrong Oncolink Survivorship Care Plan has been developed to provide such information, thus empowering cancer patients in USA to become more confident and skilled in managing their condition post-treatment.

The National Cancer Survivorship Initiative, in which Breast Cancer Haven takes an active role, aims to improve the ongoing services and support for those living with, and beyond, cancer and would like to hear your views about whether this (or a similar tool) could work in the UK.
